Due to expansion of our complex care service, we are now recruiting a Clinical Lead for our Ballykelly branch.  The post holder will be responsible for the delivery of an efficient and effective complex care service.

Clinical Lead in Complex Care is responsible for overseeing the delivery of high quality Service User care and implementing best practices and guidelines to ensure clinical standards are maintained

JOB SUMMARY

  • Overseeing the delivery of care to Service Users with complex needs
  • Manage a team of complex homecare staff
  • Coordinate clinical activities
  • Provide support and guidance to complex homecare staff
  • Regularly review and update care plans
  • Evaluate clinical outcomes to identify areas of improvement
  • To work within GDPR legislation

MAIN RESPONSIBILITIES

  1. Accept, allocate and process referrals for new complex care packages
  2. Accept and process changes and amendments to existing complex care packages
  3. Prepare rotas by matching Service Users to complex homecare staff, ensuring that the care skills and competencies are compatible 
  4. Monitor and report changes in Service User condition to key worker
  5. Organise and lead staff meetings
  6. Manage rotas, prepare paperwork for payroll and notify payroll of variations as appropriate, via the relevant process
  7. Undertake Service User assessment and risk assessment 
  8. Maintain accurate and confidential records in accordance with company policy
  9. Assist with the monitoring an evaluation of service provision
  10. Assist Registered Manager in the recruitment of complex homecare workers
  11. Identify training and development needs within their team of complex homecare workers and ensure induction training for new employees
  12. Ensure Service User and care staff meet in accordance with company policy and procedures and ensure dignity, respect, equality, and diversity
  13. Record and report to the Registered Manager any compliments, complaints, accidents, incidents and untoward events and complete necessary documentation
  14. Assist the Registered Manager and Human Resources Manager in processes, when required
  15. Provide care package to Service Users with complex need in an emergency situation
  16. Participate in mandatory training as required
  17. Ensure area PMMRs are within company benchmark
  18. Ensure Services are managed in compliance with RQIA standards
  19. Ensure an effective, safe and well led service delivery
  20. Cover the on-call phone on a rota basis and cover rotas on the ground, as and when necessary
  21. The role of Clinical Lead – Complex Care is part of the business continuity plan and job role may change according to business needs
  22. You may be required to coordinate other areas of the business, as business needs dictate
